Diagnostic Visit
Comprehensive Diagnostic Session
All new patients and prospective patients start with a detailed appointment where we will review your symptoms, goals, and perform a diagnostic evaluation. In this session, you will receive a thorough package of:
Comprehensive Diagnosis, reviewing your symptoms, history, labs, and imaging (we review images ourselves and do not rely on radiology reports) to arrive at a diagnosis of your condition;
Written Care Summary, which is a write-up of your condition that you can use when going to other doctors and specialists with your diagnosis and how to approach your unique care;
Care Framework for managing your unique case, summarizing your personal goals and our planned approach to your care.
2-Hour Visit, plus extensive prep and write-ups after visit | $1,250
The fee for the Comprehensive Diagnostic Session can later be credited to a Concierge Medical Care Membership.
Membership
Concierge Medical Care Membership
All concierge patients are offered a Comprehensive Diagnostic Session included in their membership. As a patient through a concierge care membership, you also receive:
A 1-hour holistic care appointment, every other month, with our care team
Unlimited On-demand concierge messaging for non-emergencies, to support with monitoring your case, tuning your treatment protocol, and providing consultation for evolving symptoms
Ongoing case management for imaging orders, lab orders including comprehensive blood panels, prescriptions, custom-designed rehab plans and/or coordination with your physical therapist
Annual Membership | $4,100 per year ($2,850 after credit, paid quarterly)
Please note: Before allowing someone to become a patient or client, our clinic requires every new prospective patient to do an initial 20-minute intake call at no charge. This helps our team understand if we can help you before you make an investment in having Dr. Gandham and our team as a part of your care team, and helps us understand if you are a good fit for how we work.
The Value of the Written Care Summary
The Gandham Family Clinic offers all of our patients, both members and patients who work with us only through the Comprehensive Diagnostic Session, an in-depth written care summary that includes:
A clear narrative of your history, diagnoses, objective findings, and plan
A practical roadmap that includes medications, rehab priorities, and recommendations
Language that helps outside providers take your concerns seriously—especially when they’re unfamiliar with these conditions
Advocacy support: helps ensure your case is not dismissed, and that you don’t have to repeat your story from scratch
This is particularly helpful when seeing local specialists who may not be familiar with these conditions, or navigating urgent care and ER visits. It allows me to assist your doctors so you can get the treatment you need.

No - We do not bill insurance for visits. This allows for longer appointment times, deeper analysis, and collaborative care planning that isn’t possible under traditional short medical care visit models. However, you can still use insurance for:
- Medications I prescribe
- Imaging and labs I order
- I can assist in obtaining prior authorization support, when applicable -
Yes - If you start with a diagnostic session and later choose to become a medical patient through a membership, you can apply the cost from your diagnostic session to your first quarterly membership payment.

No - we also work with cases of dysautonomia, POTS, MCAS, Thoracic Outlet Syndrome, CCI, and other medically complex cases.

Because all of our physicians are licensed in NY, to be seen in a medical capacity, you must reside in New York or be physically located in New York at all times of care. We do work with patients that reside outside of NY, but they must travel to NY to be seen for visits (both physically or virtually).
